Certificate of Attendance

CERTIFICATES OF ATTENDANCE REQUIREMENTS:

·        Any student planning to take the Learner/Driver tests at the DMV must complete the certificate of attendance form & submit it several days in advance. 

·        Be sure to review your information before submitting the request, as any incorrect information could delay the process.

·        Once the request is received and reviewed you will receive an e-mail that will indicate a date and time to pick up your certificate, or letting you know that more information is required from you. 

·        Please do not attempt to retrieve your certificate before the time appointed for you to do so.

·        A certificate that has been completed will expire after 30 days.  If you are unable to make your appointment for pick up, please call or e-mail Ms. Lewis at 770-787-2250 or email lewis.sophia@newton.k12.ga.us. 

·        A second request for any reason, such as an expired or lost certificate will result in a $5.00 charge.  NO EXCEPTIONS!
