Registration Requirements

For

New, Transfer, and Re-entry Students

 

* ALL STUDENTS MUST BE ACCOMPANIED BY A PARENT OR LEGAL GUARDIAN

** NO STUDENT WILL BE ENROLLED WITHOUT THE FOLLOWING DOCUMENTATION:

 

·        Proof of prior school enrollment:  Appropriate withdrawal form

·        A certified/official copy of his/her academic transcript

·        A certified/official copy of all discipline records.  If there have been no rules infractions, student must present a statement from the previous school to that effect.

·        Residency verification:  The parent/guardian MUST provide proper residency documentation such as a lease agreement, current utility bill, and /or closing statement in the case of a home purchase (documentation indicating intent to purchase will not suffice),  Residency documentation MUST be provided at the time of registration.

·        Custody/Guardianship documentation:  Proof of custody/guardianship MUST be provided if the child does not live with one or both parents.  This document MUST be provided at the time of registration.

·        Immunization Form:  All students, including foreign exchange students, must submit a Georgia Certification of Immunization Form 3231 at the time of registration.

·        Eye, Ear, and Dental form:  All students entering a Georgia Public School for the first time must have a Georgia DHR Form 3300 that has been completed within the previous 120 calendar days of registration (out-of –state forms will be accepted if dated within the previous 12 months).

·        Birth certificate:  New students only

·        Social Security Card

·        Students who will attain the age of twenty (20) on or before September 1, or who have received a high school diploma or GED will not be enrolled.

·        The school system reserves the right to convene an alternative placement committee to determine whether alternative school placement is appropriate if the student

    • Has dropped out of school for one quarter or more from ANY other secondary school
    • Is not in good standing (academic/behavioral) upon withdrawing from the previous school
    • Is under suspension or expulsion from another public or private school
    • Is entering from a Youth Detention Center or a behavioral rehabilitation program

·        An individual classified as a special education student is eligible for enrollment in appropriate education programs through age twenty-one (21) unless he/she has received a high school diploma or the equivalent
